Does Increased Hydration or Dextrose Reduce Prolonged Labor?
This study by Fong et al. (AJOG, 2017) sought to determine if increasing hydration in labor and addition of dextrose-containing fluids may reduce the risk of prolonged labor.
Randomized Controlled Trial (RCT)
274 nulliparous women in active labor with singletons were randomized to one of three groups: 250 mL/h of normal saline, 125 mL/h of 5% dextrose in normal saline, or 250 mL/h of 2.5% dextrose in normal saline. Primary outcome was length of labor from start of IV fluids to vaginal delivery. There were no differences in labor time amongst the three groups, first and second stage of labor duration, or C-section rates. Nor were any differences found in adverse effects were found between the groups, including chorioamnionitis, PPH, Apgar score or NICU admission. The authors conclude that there is no benefit to high dose fluids or addition of dextrose in the prevention of prolonged labor in nulliparous women in active labor.
