What Risk Factors Increase Likelihood of C-Section Infection?
This study by Boggess et al. (Obstetrics Gynecology, 2017) sought to determine risk factors for post C-section infection by using pre-incision extended-spectrum antibiotics.
Secondary analysis of Randomized Clinical Trial (RCT)
The original trial sought to determine if using azithromycin for an extended time period reduced infection following C-section (Tita et al., NEJM, 2016). In this study, the risk factors associated with infection were studied. Women of black race-ethnicity, a non-transverse uterine incision, duration of membrane rupture greater than 6 hours, and surgery duration greater than 49 minutes had the highest risk of infection. Prophylaxis with azithromycin was associated with decreased risk of maternal infection but infection rates in this study were still 8.8%. The authors suggest targeting these other factors may lead to future research that will improve postsurgical infection rates and resultant morbidities.
