Fetal Lung Mass – Best Predictors of Newborn Respiratory Distress?
This study by Girsen et al. (Prenatal Diagnosis, 2017) aimed to analyze the use of fetal lung mass imaging in predicting neonatal respiratory distress.
Retrospective Cohort Study
47 fetal lung mass cases were surveyed using ultrasound and MRI. Eight of these cases had respiratory distress, categorized as intubation and mechanical ventilation at birth, surgery before discharge, or extracorporeal membrane oxygenation. All cases had an initial ultrasound performed at a similar gestational age (~26 weeks for cases with respiratory distress and ~22 weeks for those without; p=0.9). The best predictors for respiratory distress were maximal volume >24.0 cm3 by MRI (PPV 60%; NPV 100%) and >34.0 cm3 by ultrasound (PPV 54%; NPV 100%). MRI and ultrasound can predict neonatal respiratory distress even before 24 week’s gestation, but measurements in the third trimester provide the highest PPV.
