Is EHR User Experience Linked to Hospital EHR Safety?
BACKGROUND AND PURPOSE:
The use of electronic health records (EHR) systems has failed to reduce medication errors, despite the claims that EHR adoption would improve patient safety
This lack of benefit may be attributed to poor EHR usability
Classen et al. (JAMA Network Open, 2023) assessed whether EHR safety performance is associated with EHR frontline user experience in a national sample of hospitals
All US adult hospitals that used both
The National Quality Forum Leapfrog Health IT Safety Measure
The ARCH Collaborative EHR user experience survey (a collaborative created to improve EHR experience throughs surveys and benchmarking)
Ordinary least squares models with survey responses clustered by hospital were used to assess associations between the overall measures and their subcomponents
Hospital safety performance
Based on the Leapfrog Health IT Safety measure (overall and 10 subcomponents)
EHR user experience scores
Based on the ARCH collaborative frontline user experience scores (overall and 8 subcomponents)
112 hospitals | 5689 frontline user surveys
Mean safety scores: 0.673 (range, 0.297 to 0.973)
Mean ARCH EHR user experience score: 3.377 (range, 1 [best] to 5 [worst])
There was a positive association between overall safety score and overall user experience score
Adjusted β coefficient 0.011 (95% CI, 0.006 to 0.016)
Overall usability was also associated with 10 safety subcategories
Overall safety was positively associated with usability subcategories
There is a positive association between EHR usability and hospital safety performance
Improving EHR usability could directly improve safety
The authors state
Both health systems and vendors need to consider usability not only as critical for the frontline users, but also as a critical safety issue in the design, development, implementation, and maintenance of these complex EHR systems, and they should work together with frontline users and organizations to improve usability without compromising the integrity of safety performance
