Is Physical Activity Linked to Reduced Mortality Among Breast Cancer Survivors?
BACKGROUND AND PURPOSE:
Chen et al. (JAMA Network Open, 2022) evaluated the association of physical activity with risk of all-cause mortality among breast cancer survivors
Postmenopausal breast cancer survivors
Initial diagnosis ≥2 years prior
Leisure time physical activity (as assessed by two validated questionnaires)
The authors used Cox proportional hazards regression model to estimate the association between activity and risk of all-cause mortality
Godin-Shephard Leisure-Time Physical Activity Questionnaire Activity Levels used to ascertain degree of activity with following examples
Active: Strenuous or frequent exercise (e.g., running, jogging)
Moderate active: Moderate exercise (e.g., fast walking, easy bicycling)
Insufficiently active/sedentary: Light and/or less frequent exercise (e.g. yoga, archery)
Adjustments: Age | Breast cancer stage | Comorbidity | Years since diagnosis | Race and ethnicity | Cancer treatment
Mean age: 71 (range 57 to 86) years
Black: 20.9% | Asian or Pacific Islander: 1.3% | Non-Hispanic White: 68.9%
Median follow-up: 7.8 (IQR, 7.3 to 8.3) years
Deaths due to any cause: 45 participants
Deaths due to breast cancer: 5 participants
Active participants: 12.9 per 1000 person-years (PY)
Moderately active participants: 13.4 per 1000 PY
Insufficiently active participants: 32.9 per 1000 PY
Risk of death among active and moderately active participants was decreased vs insufficiently active
Active: HR 0.42 (95% CI, 0.21 to 0.85)
Moderately active: HR 0.40 (95% CI, 0.17 to 0.95)
Physical activity among breast cancer survivors was associated with a 60% decreased risk of all-cause mortality, compared to those who were insufficiently active
The authors state
Our findings further suggest that survivorship care plans should consider incorporating physical activity because even moderate activity may be vital for extending survival as well as health-related quality of life
